Monthly Safety Scenario: Engine room fire caused by failed fuel pipe

Many fires are caused by hot spots not being covered by the required insulation which might have been removed during maintenance.

It is essential that the crew ensure that insulation is put back and that hot spots are identified in the engine room to prevent fires, and insulated as required. In the latest Monthly Safety Scenario, you will out what happens when the emergency quick acting valves are not properly closed.

What was the cause of this accident and how to prevent this scenario from happening again?

Read the full case in the latest MSS Case, provided by our Loss Prevention department.
