MSS Case September: At anchor during a typhoon resulting in a grounding and total loss

In the Monthly Safety Scenario Case of September, the importance of the crew being aware of the equipment limitations is being highlighted. In this case the anchoring equipment was overloaded in a typhoon, despite the effort to find a suitable place to ride out the typhoon.

A tanker had anchored in a bay outside an Asian port waiting for a berth to discharge its cargo. There had been weather warnings about an approaching typhoon. The Master considered the anchorage, surrounded by some islands, would be a suitable place to ride out the approaching typhoon.

The increasing winds caused the anchor to drag and with a total of 9 shackles payed out, the windlass did not manage to heave the anchor.
