MSS Case December: Dangerous heaving line caused injury

In the Monthly Safety Scenario Case of December, an incident during a mooring operation caused a line handler injury. This case gives advice on how to prevent personal injury to those receiving heaving lines, including permitted adaptations to onboard equipment.

Both mooring teams had prepared the mooring ropes when the vessel was approaching the berth. An AB called to the line handler that he was throwing the heaving line for the head lines. The line handler shouted back that he was ready.

Then the AB threw the heaving line but unfortunately the monkey fist hit the line handler on the head, and he had to be taken to hospital in an ambulance. Later on, the police found out that the monkey fist of the heaving line had extra weight added.

Each month the Club's Loss Prevention team issues a new safety scenario to assist members in their efforts to comply with international safety regulations and to follow best practice.

When discussing the case please consider that the actions taken at the time made sense for all involved. Do not only judge, but also ask why you think these actions were taken and could this happen on your vessel?
