Below please find the U.S. Coast Guard’s “Safety Alert 4-06” E-mail dated the 10th of May 2006, and entitled “Recall – Pains Wessex White Collision Warning (MK7) Hand Flare – Item No. 52651.”
The “Safety Alert” is also attached in PDF format. According to the “Safety Alert,” the hand flare’s manufacturer, Pains Wessex, has recalled the hand flare because of “…a recent incident in which an individual was seriously injured while using the White Hand Flare. The company has identified that there may be a risk of misfire and is recalling all white flares to prevent further injury.”
Furthermore, the USCG “…strongly recommends that all vessel owners and operators who have this equipment onboard their vessels follow the recommendations per the Pains Wessex recall notice. Additional information is available at https://www.mcmurdo.co.uk/.”
