Accidents are generally caused by human error and often occur in conjunction with technical or equipment failure. One of the recurring problems identified in this publication, is that proper procedures have not been put in place or those procedures that do exist have not been followed correctly.

In this publication you will find a number of measures you can adopt to prevent casualties from happening. To make this study and analysis conclusive, we have narrowed down the types of vessels to bulker, container and tanker. These vessels represent 80% of the Clubs insured vessels.
